Johnsburg Chiropractic & Wellness Center is NOT contracted with insurance and is an out-of-network provider. Office fees are payable at the time of your visit. We will provide you with a superbill that you can use to submit to your insurance, if it is a PPO plan. Most flex spending and health savings accounts will accept the superbill also.

We would be happy to verify your chiropractic benefits with your insurance. If you wish for us to do that, please make sure to bring your insurance card to your appointment. You can also call your insurance company with the above CPT codes to see what the out-of-network reimbursement may be.

**MEDICARE PATIENTS** please note that we will submit your claims to your insurance for you as a non-participating provider. All office fees are due at the time of service. Please bring your insurance cards with you to your appointment. Medicare or your Medicare Replacement plan would reimburse you directly as we do not accept assignment of benefits.
